testimony and/or considered the pleadings, papers, affidavits and other papers
filed i~crein, and being otherwlsc fully and well advised in the premises, it is
ORDERED AND ADJUDGED that the minor child(ren)

is/are declared to be the legitimate child(ren) of tl~e Defendant

Said amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement knit, 1317 aine~?aod Boulevard,
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJl;DCED that tine Clerk c•f the Circuit Court shall and is
hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments frog
tl~e Department of Health and RehabilitaCive Services, in which event the support
payments shall therea[ter be directed and payable to the aforesaid natural mother or
person-having custody of the child(ren); it is further
