and all parties having received proper and timely notice; the Court having heard
testimony and/or considered the pleadings, papers, affidavits and other papers
filed herein, and being otherwise fully and yell advised in the premises, it is
ORDERED AND ADJUDGED that the minor child(ren)
LARONDA DEVITA NAYES, d.o.b. 8-2-72
is/ale declared to be the legitimate child(ren) of the Defendant

the natural mother; it is further
ORDERED AND ADJUDGED that the natural ~ma~kte~arx grandmother
ALICE WHITE , shall Dave custody of the said child(ren)
subject to the Defendant s right of reasonable visitation; it is further

the Defendant/Father shall pay chilcl support for and on behalf of the said child(ren)
in the amount of $ 20.00 per week plus $2.00 statutory fee

Said amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Ci~ild Support Enforcement Unit. 1317 i.'inevaod 3oulevard,
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGED that tl~e Clerk ~f the Circuit Court shall and is
hereby ordered to continue to transmit support payments received from the Defendant
until-further order of this Court ur receipt of Notice to Discontinue Payments from
tl~e Department of Health and Rehabilitative Services, in which event the support
payments shall thereafter be directed and payable to the aforesaid natural mother or
person having custody of the child(ren); tt is further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the abo~~e-named child(ren), the DF.FARTMENT OF HEALTH AND REHABILITATIVE
SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall ar~d it is hereby ordered to

above-named father's name.
