THIS CAUSE having come on for trial upon the pleadings filed herein
and all parties having received proper and timely notice; the Court having heard
testimony and/or considered the pleadings, papers, affidavits and other papers,
filed herein, and being otherwise'fulty and well advised in the premises, it is
ORDERED AND ADJUDGED that the minor child(ren)
ROBIN DENISE McMILLAN d.o.b. 9-30-79
is/are declared to be the legitimate child(ren) of the Defendant
KENNETH ISAIH ZACKERY , and ELIZABETH D. McMILLIAN
the natural pother; it is further
ORDERED AND ADJUDGED that the natural mother,
ELIZABETH D. McMILLIAN , shall have custody of the said child(ren)
suD~ect to the Defendant s right of reasonable visitation; it is further
ORDERED AND ADJUDGED that commencing on /~A~ N / 5~ _ , 1980,
the Defendant/rather shall pay child sccpport for and on behalf of the said child(ren)
in the amount of S 104.00 per month plus X2.00 statutory fee

Said amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Cicild Support Enforcement Unit, 131,' Winewood Aoulevard,
Ta~lahassee, Florida, 32304; it is further
ORDERED AND ADJUDGED that tl?e Clerk ~f the Circuit Court shall and is
hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments Erop
the Department of Health and Rehabilitative Services, in which event the support
payments shall thereafter be directed and payable to the aforesaid natural pother or
person-having custody of the child(ren); it is further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIVE
SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to

above-named father's name.
