THIS CAUSE having come on for trial upon the pleadings filed herein
and all parties having received proper and timely notice; the Court having heard
testimony and/or considered the pleadings, papers, affidavits acxi other papers
filed herein, and being otherwise fully and well advised in the premises, it is
ORDERED AND ADJUDGED that the minor child(ren)
LAWANA WASHINGTON, d.o.b, 3-7-71
is/are declared to be the legitimate child(ren) of the Defendant
EUGENE WHITE , and CLARE WASHINGTON ,
the natural mother; it is further
ORDERED AND ADJUDGED that the natural mother,
CLARA WASHINGTON , shall Dave custody of the said child(ren)
subject to the Defendant s right of reasonable .visitation; it is further
ORDERED AND ADJUDGED that commencing on _ /77A.e~~'r1 , 1980,
the Defendant/I~atl~er shall pay child support for and on behalf of the said child(ren)
in the amount of $ 80,00 per month plus $2.00 statutory fee

Said amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, child Support Enforcement Unit, I3i7 Winewrod Boulevard,
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGED that tl~e Clerk ~f the Circuit Court shall and is
hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments fro4
the Department of Health and Rehabilitative Services, in which event the support
payments shall thereafter be directed and payable to the aforesaid natural mother or
person-having custody of the child(ren); it is further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIVE
SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall ai:d it is hereby ordered to
