testimony and/or considered the pleadings, papers, affidavits and other papers
filed herein, and being otherwise fully and well advised in the premises, it is
ORDERED AND ADJUDGED that the minor child(ren)
CEIIRIC ANTHONY COOK. d.o.b. 11-11-67; ALICE LEVERN COOK, d.o.b. 1-20-66
is/are declared to be the legitimate cl~itd(ren) of tl~e Defendant
PAUL LUNDY ~ , and SHIRLEY CAVERN ELLIS ,
the natural mother; it is further
ORDERED AND ADJUDGED that the natural mother,
SHIRLEY CAVERN ELLIS , shall have custody of the acid child(ren)
subject to the Defendant s right of reasonable visitation; it is further
ORDERED AND ADJUDGED that commencing on /YJq~(~N /t~ , 1980,
the Defendant/Father shall pay child support for and on behalf of the said child(ren)
in the amount of $ 60.00 pcr biweekly , plus X2.00 statutory fee
All payment shall be made in cash. money order or cashiers check. All money orders. •

Said amount shall~be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Ninewood Boulevard,
Tallahassee, Florida, 32304; it is further -
ORDERED AND ADJUDGED that tl~e Clerk ~f the Circuit Court shall and is
hereby ordered to continue to transmit s~~pport payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Pa}~raents from
tt~e Department of Health and Rehabilitative Services, in which event the support
payments shall thereafter be directed and payable to the aforesaid natural mother or
person-having custody of the child(ren); it is further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DF.FARTMENT OF HEALTH AND REHABILITATIVE
SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall ai?d it is hereby ordered to
