THIS CAUSE having come on for hearing on the petition of the DEPARI?tENT
OF HEALTH AND REHABILITATIVE SERVICES for child support and the Court having found
that said Petitioner is a proper party by virtue of an assignsent of rights to
child support signed by the party having custody of the dependent child(ren) and
all parties having received proper notice, it is
ORDERID AND ADJUDGID that the Petition for Child Support shall be and
it is hereby granted; it is further
ORDERED AND ADJUDGED that coaseencing on !Y)A~e['H 1980,
Lhe responsible parent HERMAN HILLMAN FELTON ~ who is the father ,
of the minor child(ren), to-wit: LAQUANNA FELTON d.o.b. 5-14-78
shall pay to the Clerk of this Court the sum of S 25.00 per month each and
every month plus $2.00 statutory fee, until the AFDC arrears are reimbursed
~ n rh amount of S4S3 All payments shall be made in cash,
money order or cashiers c ecc. ll~money orders and cashiers checks shall bear the
payees name and Social Security Number and shall be made payable to the CLERK OF THE
~ CIRCUIT COURT, and sent to:

Said amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard,
Tallahassee, Florida, 32304. It is further
ORDERED AND ADJUDGID that the Clerk of the Circuit Court shall and (s)he
~ is hereby ordered to contirn,e to transmit support payments received from the Respondent
to the Department of Health and Rehabilitative Services until further order of this
Court to the contrary or receipt of Notice to Discontinue Payments from the Department
of Health and Rehabilitative Services, in which event the support payments shall tMre-
after be directed and payable to the person having custody of the child(ren).
