HomeMy WebLinkAbout1328 . . 4'794'76 nircctor of Utilities Fort Pierce Utilities Authority ti~inicipll Building 206 Soieth nth Street Fort Pierce, Florida 33450 Tear Sir : - The undersigned does hereby apply to the Fart Pierce Utilities Autlioz•ity for an extension of water service outside the City Limits of the City of Fort Pierce, Florida, to the following describ©d property which I own, to-wit: Lot 3 less the west S feet of Block 4 of Dark Hammock Estates, Unit 1, as per plat recorded in Plat Book 14, Page 13, Public Records of St. Lucie County. The r.~ailing address of the above described property is: 601 Dark Hammock Road Fort Pierce, Florida, 33450 I understand that in order-to obtain extension of such water service,~that I will be required to pay the cost of construction of such water line from the point of connection ~tiith existing lines of the Utilities Authority to my property, that the total of such cost shall be deposited with the Utilities Authority prior to the time of the construction of said line, that I ~~~ill be obligated to pay all meter fees and connection charges required under the Resolutions of the Utilities Authority for such service priox to the installation of said line, and that I will pay, wren billed, for all water furnished to my property according to the rates established by the Utilities Authority for the sere*ice I receive. In addition to the deposit of said construction cost as set forth in the ati.~ove paragraph, I understand that upon execution of this application, I am required to deposit with the Utilities-Authority an amount equal to ten (10~) percent of the connection fees for water service for each unit to be served to and including twenty-five (25) units, and five (5`~0) percent of such connection fees for each unit in excess of twenty-five (25) units. In the event this application is not approved by the Utilities Authority and the City Conunission of the City of Fort Pierce, then such deposit will be returned to me. If this - application is approved by the Utilities Authority and the City Corrvnission of the City of Fort Pierce, the deposit of the connection fees as above outlined shall he credited to the total cost of the connection fees. I further understand that if this application is approved b3' the Utilities Authority and theCity Co:~~.•nission of the City of Fort Pierce, Florida, and I have not completed construction of the grater line requested within one year from the date of the approral bS- the City Co:mnission of t~~e City of Fort Pierce, Florida, then such appro~=al s,~all become null and void and it will become necessar.}• for rie to initiate a ne:v application for such water service. I understand that the extension of said ~:~ater service to my p~•o.~ei•ty shall be contingent upon the abilitl? of the Utilities Authority- to supply the deriand requested by me without in any way interfering with the crater services now performed bl the Utilities Authorit}~ for the intz<ibitants of the City of Fort Pierce, Florida. - I herc;by agree that in consideration of m}• request f~~r the _ c:;tension of «•ater service to my property under the conditions above r~;~:ntioned, that if my property becomes contiguous to the City Limits af' the City of Fort Pierce, Florida, as such term is defined in the 2a«•s of Fl~~rid~~, then-this application shall be considered as my application for BQO I ~ Pt~f - i ~ z-
