Dear Sir:
The undersigned does hereby apply to the Fort Pierce Utilities
Authority for an extension of water service outside the Cittr Limits of
the City of Fort Pierce, Florida, to the following described property
which I own, to-wit: ~
Lot 10 and South 15 feet of Lot 9 Blakely Subdivision
The mailing address of the above described property is:
1806 North 16th Court
I understand that in order to obtain extension of such ti?ater
service, that I will be required to pay the cost of construction of
such water line from the point of connection with existing lines of the
Utilities Authority to my property, that the total of such cost shall
be deposited with the Utilities Authority prior to the time of the t
construction of said line, that I will. be obligated to pay all meter
fees and connection charges required under the Resolutions of the f
Utilities Authority for such service prior to the installation of said
line, and that I will pay, wren billed, for all water furnished to my f
property according to the rates established by the Utilities Authority 4
for t:~e service I receive. 3
In addition to the deposit of said construction. cost as set
forth in the above paragraph, I understand that upon execution of this
application, I am required to deposit with the Utilities Authority an
amount equal to ten~(10`~) percent of the connection fees for water i
service for each unit to be served to and including twenty-five (2~)
units, and five (5 n) percent of such connection fees for each unit in
excess of twenty-five (25) units. In the event this application is not
approc?ed by t!ie Utilities Authority and the City Commission of the City
of Fort Pierce, then such deposit will be returned to me. If this
application is approved by the Utilities Aut}ZOrit1• and the City Corvnission f
of the City of Fort Pierce, the deposit of tl~e connection fees as above
outlined shall be Credited to the total cost of the connection fees.

I further understand that i f this application is approved bS- the
L'til ities Authority and. ,the Eity Commission of Lhe City of Fort Pierce,
Florida, and I have not completed construction of the ~sater line requested
~l:ithin one year from the date of the approval bl the Cite Co~nr.~ission of
t!~e City of Fort Pierce; Florida, then such appro~-a1 s'~all become null
and void and it will become necessary for r!me to initiate a ne« application
for such water service. .
I understand that the extension of said pater service to my
pro;~erty shall be contingent upon the ablitt- of the utilities Authority
to supply the denand requested by me without in am ~.ay interfering with
the water services now performe3 by the Utilities Authority for the
inhabitants of the City of Fort Pierce, Fl~rid<<.
I hereby agree that in consideration of m~- request for the
extension of water servi.c4 to ray property under thc~ conditions above
mentioned, that if my property becomes contiguous to the Citt• Limits of
the City of Fort Pierce, Florida, as such term is defined in the lao-s of
Florida, then this appllCatiOn shal_1 be considered as mj• application for
