Dear Sir:
The undersigned does hereby apply to the Fart Pierce Utilities {
Authority for an extension of water service outside the City Limits of
the City of Fort Pierce, Florida, to the following described property

I understand that in order to obtain extension of such water
service, that I will be required to pay the cost of construction of
such water line from the point of eonnection_~vith existing lines of 'the
Utilities Authorit;? to my property, that the total of such cost shall
be deposited with the Utilities Authority prior to the time of the
construction of said line, that I :vill be obligated to pay all meter
fees_and connection charges required under the Resolutions of the
Utilities Authority for such service prior to the installation of said
line, and that I will pay, ~r:~en billed, for all water furnished to my
property according to the rates established by the Utilities Authority
for t:~e service I receive.

In addition to the deposit of said construction cost as set
forth in the above paragraph, I understand that upon execution of this
application, I am required tQ deposit with the Utilities Authority an
amount equal to ten (lOn) percent of the connection fees for water ,
service for each unit to be served to and including twenty-five (25)
units, and five (50) percent of such connection fees for each unit in t
excess of twenty-five (25) units. In the event this application is not
approved by the Utilities Authority and the City Commission of the City
of Fort Pierce, then such deposit will be returned to me. If this
application is approved by the Utilities Authority and the City Commission
of the City of Fort Pierce, the deposit of the connection fees as above
outlined shall be credited to the total cost of the connection fees.
I further understand that if this application is approved by the
Utilities Authority and the City Commission of the City of Fort Pierce,
Florida, and I have not completed construction of the water line requested
~cithin o~~e year from the date of the approval by the City Co:nr:~ission of
the Cicy of F~i•t Pierce, Florida, then such approval shall become null
a.~d void and it will become necessary for me to initiate a new application
far such water service.
I understand that the extension of said water service to my
property shall be contingent, upon the a'oility of the Utilities Authority
to supply the denand requested by me without in any way interfering with
the water services now performed by the Utilities Authority for the
inhabitants of the City of Fort Pierce, Flot•ida.
I hereby agree that in consideration of mt~ request for the
extension of water service to r.~y property under the conditions above '
mentioned, that if my property becomes contiguous to the City Limits of
the City of Fort Pierce, Florida, as such ter~r is defined in the laws of
Florida, then this application shall be considered as my application for
