The mailing address of the above described property is: '007 I~vc- ,.,h FJ_ ~ir~c.c ~,t q ,~.~yf o t I understand that in order to obtain extension of such water = service, that I will be required to pay the cost of construction of such water line from the point of connection with existing lines of the _ Utilities Authority to my property, that the total of such cost shall be deposited with the Utilities Authority prior to the time of the construction of said line, that I will be obligated to pay all meter } fees and connection charges required under the Resolutions of the Utilities Authority for such service prior to the installation of said - line, and that I will pay, when billed, for all water furnished to my property according to the rates established by the Utilities Authority for the service I receive. In addition to the deposit of said construction cost as set forth in the above paragraph, I understand that upon execution of this application, I am required to deposit with the Utilities Authority an amount equal to ten (10`x) percent of .the connection fees for water s service'for each unit to be served to and including twenty-five (25) units, and five (5p)-percent of such connection fees for each unit in excess of tiveniy-five (25) units. In the event this application is not approved by the Utilities Authority' and the City Commission of the City of Fort-Pierce, then such deposit will be returned to me. If this application is approved by the Utilities Authority and the City Commission of the City of Fort Pierce, the deposit of the connection fees as above outlined shall be credited to the total cost of the connection fees. I further understand that if this application is approved by the Utilities Authority and the City Com.-nission of the City of Fort Pierce. Florida, and I have not completed construction of the water line requested within one year from the date of the ap~rova 1 by the City Commission of the City of Fort Pierce, Florida, then such approval wall become null and void and it will become necessary for me tv initiate a new application for such water service. I understand that the extension of said ~~~ater service to my prvnerty shall be contingent upon the ability of the Utilities Authority to supply the denand requested by~rrle without in any way interfering with the water services now performed by the Utilities Authority for the inhabitants of the City of Fort Pierce, Florida. I hereby agree that in consideration of my request for the extension of water service to my property under the conditions above mentioned, that if my property becomes contiguous to the City Limits of the City of Fort Pierce, Florida, as such term is defined in the laws of.
