The undersigned does hereb}• apply to the Fort Pierce Utilities
Autl~oi•ity for an extension of water service outside the City Limits of ~
the City of Fort Pierce, Florida, to the following described property

I understand that in order to obtain extension of such water
ser~,•ice, that I will be required to pay the cost of construction of
such water line from the point of connection with existing lines of the
Utilities Authorit~ to my property, that the total of such cost shall
be deposited with the Utilities Authority prior to the time of the
construction of said line, that I will be obligated to pa}• all meter
fees and connection charges required under the Resolutions of the o
Utilities Authority for such service prior to the installation of said
line, and that I will pay, wren billed, for all water furnished to my 3
property according to the rates established by the Utilities 9uthorit}'
for t:ie service I receive.
In addition to the deposit of said construction cost as set
forth in the above paragraph, I une',erstand that upon execution of this 3
application, I am required to deposit kith the Utilities authority an
a~,ount oyual to ten (10`,'x) percent of the connection fees for cater
service for each unit to be served to and including twent}~-five (25)
tin its, and five (5p)"percent of such connection fees for each unit in ~
excess of twenty-five (25) units. In the event this application is not ;
approved by the Utilities Authority and the City Com,-nission of the City
of Fort Pierce, then such deposit will be returned to me. If this
~ application is approved by the Utilities Authority and the City Commission
t ~f the City of Fort Pierce, the deposit of the connection fees as above

outlined shall be .credited to the total cost of the connection fees.

E I further understand that if this application is approved b}• the

Utilities Authority and the City Commission of the City of Fort Pierce,
1•'lorida, and I have not completed construction of t'_ie cater line requested
t:~ithin one year from the date of the- approval b}• the City Co:nriission of "
the City of Fort Pierce, Florida, then such approval s~~all become null
and void and it will become necessary for r!me to initiate a new application
for such water service_
~ I understand that the extension of said cater service to m}•
hronerty shall be contingent upon the abilit}~ of the '.itilities Authority
to supply the denand requested b}- me without in am way interfering with
the water services now performled_by the Utilities Authorit}• for the
inhabitants of the City of Fort Pierce, Florida.
I hereby agree that in consideration of m}" request for the
extension of water service to my property under the conditions above
mentioned, that if my property becomes contiguous to the Cit}• Limits of
the City of Fort Pierce, Florida, as such term is defined in the laws of
Florida, then this application shall be considered as my application for
