THIS CAUSE having come on for trial upon the pleadings filed herein
and all parties having received proper and timely notice; the Court having heard
testimony and/or considered the pleadings, papers, affidavits and other papers
filed herein, and being otherwise fully and yell advised in the premises, it is
ORDERED AND ADJUDGED that the minor child(ren)
JEREMIAH HUBERT THURSTON, d.o.b. 3-7-71; CHRISTOPHER FRED MAJOR, d.o.b. 7-10-69
is/are declared to be the legitimate child(ren) of the Defendant
CHARLES THURSTON , and ALICE MAJOR a/k/a ALICE SAINTIL ,
the natural mother; it is further
ORDERED AND ADJUDGED that the natural mother,
ALICE MAJOR a/k/a ALICE SAINTIL , shall Dave custody of the said child{ren)
subject to the Defendant s right of reasonable visitation; it is further
ORDERED AND ADJUDGED that commencing on ~`~4, = ~ , 1980,
the Defendant/Father shall ply child suppor for and on behalf of the said child{ren)
in the amount of $ 3 O. y CJ per ~e~ plus X2.00 statutory fee
until the AFDC is reimbursed ifi the amount of $2122.00 .
All payment shall be made in cash. money order or cashiers check. All money orders
and cashiers checks shall bear the payees name and Soc1a1 Security Number and alull
be made payable to tite CLERK OF THE CIRCUIT COURT, and •ent to:

Said amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit. 131? Winewood Boulevard,
Tallahassee, Florida. 32304;~it is further
ORDERED AND ADJUDGED that the Clerk ~f the Circuit Court shall and is
hereby ordered to continue to trAnsmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments frog
the Department of Health and Rehabilitative Services, in which event the support
paytents shall thereafter be directed and payable to the aforesaid natural mother or
person-having custody of the child(ren); it is further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIVE
SERVICES, BUREAU OF VITAL STATISTICS, APfENDMENT UNIT, shall and it is hereby ordered to
