THIS CAUSE having come on for trial upon the pleadings filed herein.
and all parties having received proper and timely notice; the Court having heard
testimony and/or considered the pleadings, papers, affidavits and other papers
filed herein, and being otherviae fully and well advised in the premlises, it is
_ ORDERED AND ADJUDGED that the minor child(ren)
ANTONIO DEMETRIUS BUSH, d.o.b. 12-26-76
is are declared to be the legitimate child(ren) of the Defendant
RANDOLPH CHRISTOPHER YOUNG and TYLESIA BUSH ,
the natural mother; it is further
ORDERED AND ADJUDGED that the natural mother,
TYLESIA BUSH shall have custody of the said child(ren)
subject to the Defendant s right of reasonable visitation; it is further
ORDERED AND ADJUDGED that commencing on March 14 , 1980,
the Defendant/Father shall pay child support for and on behalf of the said child(ren)
in the amount of $ 15.00 per week , plus-$2.00 statutory fee

All payment shall be made in cash. money order or cashiers check. All monev orders ,
and cashiers checks shall bear the payees name and Social Security Number and alwll
be male payable to the CLERK OF THE CIRCUIT COURT, and sent to:

Said amount shall be remitted monthly by the Clerk to the Departmlent of Health and
~ Rehabilitative Services, Child Support Enforcement Unit, 1317 iiinevood Boulevard,
Tallahassee, Florida, 32304; it is further
. ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and is
hereby ordered to continue to transmit support payments received from the Defendant
until further order of thie Court or receipt of Notice to Discontinue Payments froo
- the Department of Health and Rehabilitative Services, in ~+hich event the support
payments shall thereafter be directed and payable to the aforesaid natural mlother or
person•having custody of the child(ren); it is further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIVE
SERVICES, BUREAU OF VITAL STATISTICS,.AMENDMENT UNIT, shall and it is hereby ordered to
