PETITION FOR APPOINTMENT OF GUARDIAN
The undersigned petitioners, JOHN WILLIAM MEEHAN, SR. and
FLORENCE REGINA MEEHAN, his wife, allege as follows:
1. Petitioners' residence and post office address is: 418
S.E. Verada Avenue, Port St. Lucie, St. Lucie County, Florida.
2. MICHELLE MARIE JOHNSTON is an incompetent, whose date
of birth was March 11, 1968, and who is twelve (12) years of age.
The residence and post office address of the incompetent is: -418 S.E.
Verada Avenue, Port St. Lucie, St. Lucie County, Florida.
3. The nature of the incompetent's incapacity is minority.
4. It is necessary that a guardian of the person and of
the property be appointed for the incompetent.
S. The approximate value and description of the incompetent's
property are presently unknown to the Petitioners.
6. The names and addresses of the persons most closely
related to the incompetent are, to the best of the Petitioners' know-
