The unders~ned petitioner alleges:
1. Petitioner's residence is 5984 Banana Road, West Palm Beach,
Florida 33406 and petitioner's post office address is the same
2. MICHELLE MARIE JOHNSTON is an incompetent,
whose date of birth was March 11, 1968 ,who is 12 years of age,
ajld whose social security number is . unknown .The residence of the incompetent is
167 Floresta Drive, Port St. Lucie, Florida and the post office
address of the incompetent'is the same _
She may also be temporarily residing at 418 S.E. Verada Avenue,
Port St. Lucie, Florida with Mr. and Mrs. Jack Meehan
3. The nature of the incompetent's incapacity is her minority of age, and _

Her surviving parent Larry Frederick Johnston, died March 10, 1980.
4. It is necessary that a guardian of the -pew-son-- aid-p=e$~.,
be appointed for the incompetent.

some furniture and personal property having a value of approximately '
$2,000.00. She may be entitled to social security survivor's benefits
and insurance benefits the existence and amount of which are unknown
to Petitioner now.
