LIBERTY MUTUAL INSURANCE COMPANY, a Workers' Compensation
i~t Carrier for the Employer of Plaintiff, MARY ANN PRESTON, pursuant

to 440.39 (3) (a) of the Florida Statutes, gives notice that
NARY ANN PRESTON was in the course of her employment with the
employer, AMERICAN AUTOMOBILE ASSOCIATION, when she was injured
at the time and place alleged in the Complaint filed in this
cause, and LIBERTY MUTUAL INSURANCE COMPANY has paid to the
I' Plaintiff, MARY ANN PRESTON, various sums of money for Workers'
Compensation and medical benefits and that it is probable that .
j future compensation and medical benefits under the Florida Workers'
Compensation Act will be paid to the Plaintiff.
WHEREFORE, LIBERTY MUTUAL INSURANCE COMPANY claims a lien
upon any recovery by the Plaintiff for 50$ of the .amount of°``,
benefits previously paid to the Plaintiff under Florida Worker's.
Compensation Act, as well as 50$ of the amount of benefits to be
paid in the future under the provisions of the Act.
