because Susan Smythers and William Foreman have waived their
right to act and consented to the appointment of your petitioner.

petitioner is unaware of any unrevoked wills or codicils of
decedent.

CHESTER B. GRIFFIN, a resident of St. Lucie County, Florida,
as her agent or attorney for service of process in any suit or
action against her in her representative capacity, or per-

Petitioner requests that she be appointed personal
representative of the estate of the decedent.
