The undersigned petitioner alleges:
1. Petitioner's residence and post office address is:
2050 Oleander Avenue, Fort Pierce, Florida 33450.
2. THOIK,AS E. HALL is an incompetent, whose date of birth
is January 12, 1915, which is 65 years of age, aid whose social
security number is 263-07-0701. The residence and post office address
of the incompetent is 302 North 14th Street, Fort Pierce, Florida 33450.
3. The nature of the incompetent's incapacity is a left,
cerebral vascular accident, diabetes, hypertension and renal disease,
as adjudged by the Circuit Court in St. Lucie County, Florida on
April 22, 1980.
4. It is necessary that a guardian of the person and
property be appointed for the incompetent.

j incompetent's property are presently unknown to the Petitioner.
6. The names and addresses of the persons most closely
related to the incom etent are resentl unknown to the Petitioner.
