THIS CAUSE having cove on for hearing on the petition of the DEPARI?tENT
OF HEALTH AND REHADILITATI_VE SERVICES for child support and the Court having found
that said Petitioner is a proper party by virtue of an assignment of rights to i
child support signed by the party having custody of the dependent child(ren) and
all parties having received proper notice, it is
ORDERED AND ADJUDGED that the Petition for Child Support shall be and
it is hereby granted; it is further
ORDERED AMID ADJUDGED that commencing on AP~e/L ~8 , 1980,
the responsible parent LEON EDWARD HILL who is the father ~
of the minor child(ren), to-wit: LEON E. HILL, JR., d.o.b. 4-2-64; GERALD J. HILL,
d.o.b. 6-24-65; VINCENT W. HILL, .o.b. - - EL .o. - -
THEODORE L. HILL, d.o.b. 9-30-73; TERESA A. HILL, d.o.b. 4-2-75

shall pay to the Clerk of this Court the sum of $ 50.00 per biweeklyeach and
every biweekly , plus $2.00 statutory fee, until the arrearage in the amount of

Said amount shall be remitted monthly by the Clerk to the Department of Health and F
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard,
Tallahassee, Florida, 32304. It is further
ORDERED AND ADJUDGID that -the Clerk of the Circuit Court shall and (t)he
is hereby ordered to continue to transmit support payments received from the Respondent
to the Department of Health and Rehabilitative Services until further order of this
Court to the contrary or receipt of Notice to Discontinue Payments frog the Department
of Health and Rehabilitative S_ervicea, in which event the support payments shall there-
after be directed and payable to the person having custody of the child(ren). ~
