Said amount shall be remitted monthly by the Clerk to the Departpent of Healt and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Doulev,~,
Tallahassee, Florida, 32304; it is further -
ORDERED AND ADJUDGED that the Clerk ~f the Circuit Court shall and is
hereby ordered to continue to transmit support payments received frog the Defendant
until further order of .this Court or receipt of Notice to Discontinue Paypents from
tl~e Departsient of Health and Rehabilitative Services, in which event the support
payments shall thereafter be directed and payable to the aforesaid natural atother or ~
person-having custody of the child(ren); tt is~further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF HF~ILTH AND REHABILITA?IVE
SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to:
