Said avount shall be rewitted aanthly by the Clerk to the Departwent of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard,
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGED that the Clerk ~f the Circuit Court shall and is
hereby ordered to continue to transmit support payments received frow the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments Erow
tl~e Departaent of Health and Rehabilitative Services, in which event the support
paysents shall thereafter be directed and payable to the aforesaid natural siother or
person-having custody of the child(ren); it is further
ORDERID AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF HF~LTH AND REHABILITATIVE
SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to:
