Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard, i
Tallahassee, Florida,, 32304; it is further
ORDERED AND ADJUDGED that•the Clerk ~f the Circuit Court shall and is
hereby ordered to continue to trana~it support payments received frow the Defendant
until further order of this Court or receipt of Notice to Discontinue Paywents from
the Departwent of Health and Rehabilitative Services, in which event the support
paysknts shall thereafter be directed and payable to the aforesaid natural wother or'
person-having custody of the child(ren); it is further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITA?IVE
SERVICES, BUREAU OF VITAL STATISTICS, AI~NDMENT UNIT, shall and it is hereby ordered to:
