AFFIDAVIT OF OPERATORS USING TRADE NAME
STATE OF FLORIDA
COUNTY OF ST. LUCIE
BEFORE ME, the undersigned authority, this day
personally appeared DOMINIC J. ALBI, who being by me first duly
sworn, deposes and says:
That this Affidavit is made in compliance with
Section 865.09 of the Florida Statutes.
That the Affiant composes all of the persons
interested in the business enterprise known and doing business
as: DLLI SHACK, located and operating at-the following address:
8000 South Federal Highway
Grimes Plaza II
Port St. Lucie, Florida 33452
and that the extent of the interest of each Aff iant in said
enterprise is as follows:

That the Affiant has advertised his intention to
reyister the foregoing fictitious name pursuant to the provisions
of Section 865.09, Florida Statutes, at least once a week for
four consecutive weeks prior to the filing of this Affidavit in
The News Tribune, a newspaper of general circulation of St. Lucie
County, Florida published continuously in said County each week
for a period of one yuear next preceding the first publication of
said notice, all of which will. more fully appear by Publisher's
Affidavit of Proof of .said publication hereunto attached and by
reference thereto made a part of this Affi vit.
