MEDICAL PAYMENTS
FLORIDA FARM BUREAU INSURANCE COMPANIES, a Workers' Compen4a-

tion Carrier for the Employer of the Counterclaimant, HARRY KOENIG,
pursuant to 440.39(3)(a) of the Florida Statutes, gives notice
that HARRY ROENIG was in the course of his employment with the
employer, FRENZ ENTERPRISES, INC., when he was injured at the

~ time and place alleged in the Complaint filed in this cause, and
E FLORIDA FARM BUREAU INSURANCE COMPAL~IES has~paid to the Counter-
claimant, HARRY ROENIG, various sums of money for Workers' Compensa-~
t ~
~ tion and medical benefits and that it is probable that future compen~

sation and medical benefits under the Florida Workers' Compensation ~
Act will be paid to the Plaintiff.
WHEREFORE, FLORIDA FARM BUREAU IP~SURANCE COMPANIES claims a
lien upon any recovery by the Counterclaimant, HARRY KOENIG, for
~ the full amount of benefits previously paid to the Counte;claimant,
