ON BEHALF OF MINOR AND ALLOCATION OF PROCEEDS
FOR ATTORNEYS FEES AND COSTS
This matter having come on before the Court upon the Petition of JAMES .
L. BUTTS and LORETTA J. BUTTS as parents and next friends of JEANETTE M. BUTTS,
~ a minors for an Order approving settlement and payment of attorney's fees and
costs and allocation of the net proceeds of the settlement, and the Court having

considered same and being otherwise fully advised in the premises, it is
ORDERED AND ADJUDGED that Plaintiffs/Petitioners, JAMES L. BUTTS and
III LORETTA J. BUTTS, be and hereby are .authorized to accept settlement of the claim
as set forth in the Petition and proceeds thereof shall be distributed as
