PETITION FOR DISCHARGE
The undersigned, as personal representative of the above estate alleges:
1. The decedent, J~IId BOI~AND a resident
of Diilton.Ontario Canada L9T 3L2 ,died on
NowerrbPS 28 1979 ,and Letters of Administration were issued to
Petitioner on 6 19 79
2. Petitioner files herewith a final accounting containing a complete report of all
receipts and disbursements since the commencement of administration of this estate, or
since the date of the last periodic accounting filed herein, if any, or waiver of the filing and
any hearing on the same, signed by all interested persons, 'are attached hereto.
3. Petitioner has fully administered this estate, by making payment, settlement,
~•'+:r ~i~rasition of all claims and debts that viere presented, and by paying or making
provision fcr the payment of all expenses of administration.
4. Petitioner has filed all required estate tax returns with the Internal Revenue
Service end with the Department of Revenue of the State of Florida, and has obtained and
filed with this court evidence of the satisfaction of this estate's obligations for both federal
anti Florida estate taxes, if any.
I 5. Petitioner has made or proposes to make distribution of the assets of this estate

4 as snt forth in the schedule of distribution attached hereto as Exhibit A.
6. The only persons having an interest in this proceeding and their respective ad-
dresses are:
