NOTICE OF LIEN
COME NOW RINKER MATERIALS CORPORATION and LIBERTY •
MUTUAL INSURANCE COMPANY, by and through their undersigned
attorneys, and file this their Notice of Lien, pursuant to
Florida Statute 440.39, showing unto the Court the following:
1. LIBERTY MUTUAL INSURANCE COMPANY is a workers'
compensation carrier for the employer of the plaintiff, RINKER
MATERIALS CORPORATION.
2. At all times material to this lawsuit, plaintiff,
CHARLES HECKMAN, was in the course and scope of his employment
with the employer, RINKER MATERIALS CORPORATION, when said

plaintiff was injured at the time and place alleged in the
complaint filed in this cause.
i~ ~ 3. LIBERTY MUTUAL INSURANCE COMPANY has paid to the

plaintiff, CHARLES HECKMAN, various sums of money for workers'
compensation and medical benefits and it is probable that future
compensation benefits under the Florida Workers' Compensation
Act will be paid to said plaintiff.
WHEREFORE, pursuant to Florida Statute 440.39(3)(a),
LIBERTY MUTUAL INSURANCE COMPANY claims a lien upon any recovery
by plaintiff, CHARLES HECKMAN, for the full amount of benefits

previously paid said plaintiff under the Florida Workers'
Compensation Act, as well as the full amount of benefits to be
