The undersigned, EARLINE BURNS , the Petitioner in
this action for Dissolution Of Marriage, does sign and file this her
voluntary Dismissal of th3.s action without prejudice on the date stated-
herein below. The undersigned does acknowledge in filing this Voluntary ~
Dismissal that the law firm of BRAD CULVERHOUSE, ATTORNEY AT LAW, CHARTERED,
has advised the undersigned of the consequences of filing this Voluntary
Dismissal including the consequence that if the Petitioner should decide
to refile this action that the Petitioner would have to pay court costs
again and would have to pay additional attorneys fees and that the Petitioner
could be liable for court costs incurred by the other party in this action.
The undersigned also understands that once this Voluntary Dismissal is

! filed with the court that this case will be dismissed and the court will

E no longer have any power over the Petitioner or the Respondent and that any
orders entered by the-Court in this case. will be of no force or effect.- ~
The undersigned further acknowledges that the undersigned is not entitled
to a refund of any attorneys fees and the undersigned also acknowledges
that the undersigned is not entitled to a refund of any court costs which
have been expended.
