and all parties haviflg received proper and timely notice; the Court having heard
testimony and/or considered the pleadings, papers, affidavits and other papers {
filed herein, and being otherwise fully and yell advised in the premises, it is
ORDERED AND ADJUDGED that the minor child(ren)
VIRGINIA MAE TOMMIE, d.o.b. 10-2-77; KEVIN DESMOND TOMMIE, JR., d.o.b. 2-10-79 ~
is/are declared to be the legitimate child(ren) of the Defendant

ORDERID AND ADJUDGED that the natural mother,
DOROTHY CLARK shall nave custody of the said child(ren)
subject to the Defendant s right of reasonable visitation; it is further

the Defendant/Father shall pay child suppoJort for and on b alf of the said child(ren)

Said amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard, j
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGED that tl~e Clerk cf the Circuit Court shall and is
hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments from
the Department of Health and Rehabilitative Services, in which event the support i
payments shall thereafter be directed and payable to the aforesaid natural mother or t
person-hawing custody of the child(ren); it is further ~
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIVE
SERVICES, BUREAl3 OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to:.
