THIS CAUSE having come on for trial upon the pleadings filed herein
and all parties having received proper and timely notice; the Court having heard
testimony and/or considered the pleadings, papers, affidavits and other papers
filed herein, and being otherwise fully and well advised in the premises, it is
ORDERED AND ADJUDGED that the minor child(ren)
LATISA JUNDUNN MOTT, d.o.b. 7-29-73
is are declared to be the legitimate child(ren) of the Defendant
GEORGE SIKES , and LURRIE MOTT ,
the natural mother; it is further
ORDERED AND ADJUDGED that the natural mother,
LURRIE MOTT , shall have custody of the said child(ren)
subject to the Defendant s right of reasonable visitation; it is further
ORDERED AND ADJUDGED that cosmencing on , 1980,
the Defendant/Father shall pay child support for and on behalf of the said child(ren)
in the amount of S per plus X2.00 statutory fee

I Said amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Ninewood Boulevard,
Tallahassee, Florida, 32304; it is further -
ORDERED AND ADJUDGED that tlu Clerk of the Circuit Court shall and is
- hereby ordered. to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments fros
the Department of Health and Rehabilitative Services, in which event the support
payments shall thereafter be directed and payable to the aforesaid natural smother or
person-having custody of tht child(ren); it is further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren). the DEPARTMENT OF HEALTH AND REHABILITATIVE
