THIS CAUSE having come on for hearing on the pstition of the DEPARI?IENT
OF HEALTH AND REHABILITATIVB SERVICES for child support and the Court having found
that said Petitioner is a proper party by virtue of an asaign•ent bf rights to
child support signed by the party having custody of the dependent child(ren) and
all parties having received proper notice, it is
ORDERED AND ADJUDGED that the Petition for Child Support shall be and

shall pay to the Clerk of this Court the sum of $ 25.00 per week each and
every week , plus $2.00 statutory fee, until 52331.00 is reimbursed to DHRS
All payments shall be made in cash,
money order or cashiers c ec 1 money orders and cashiers checks shall bear the
~I payees name and Social Security Number .and shall be made payable to the CLERK OF THE

~ Said amount shall be remitted aanthly by the Clerk to the Department of Health and '
Rehabilitative Services, Child Support Enforcesent Unit, 1317 Winewood Boulevard,
Tallahassee, Florida, 32304. It is further
ORDERED AND ADJUDGID that the Clerk of the Circuit Court shall and (s)he
is hereby ordered to continue to tranawit support payments received from the Respondent
to the Department of Heslth~and Rehabilitative Services until further order of this
Court to the contrary or receipt of Notice to Discontinue Payments fro>. the Depart eat
of Health and Rehabilitative Services, in ~+hich event the support payments shall there-
after be directed and payable to the person having custody of the child(ren).
