THIS CAUSE having core on for hearing on the petition of the DEPARI?IENT
OF HEALTH AND RElU1BILITATIVE SERVICES for child support and the Court having found
that said Petitioner is a proper party by virtue of an assignuent of rights to
child support signed by the party having custody of the dependent child(ren) and
all parties having received proper notice, it is
ORDERED AND ADJUDGED that the Petition for Child Support shall be and
it !e hereby granted; it is further
ORDERED AND ADJUDGED that commencing on June 1 1980,
the responsible parent JOHNNY SANDERS who is .the father ,
of the ainor child(ren), to-wit:_ APRIL SANDERS. d.o.b. 10-13-71; PRECIOUS SANDERS,
d.o.b. 8-13-76
shall pay to the Clerk of this Court the sum of $ 30.00 per month each and
every month , plus $2.00 statutory fee, until $520.00 is reimbursed to DHRS
All payments shall be wade in cash,
money order or cashiers c ec 1 money orders and cashiers checks shall bear the
payees name and Social Security Number and shall beaoade payable to the CLERK OF T-HE
CIRCUIT COURT, and sent to:

Said amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Ninevood Boulevard,
Tallahassee, Florida, 32304. It is further
ORDERED AND ADJUDGID that the Clerk of the Circuit Court shall and (s)he
is hereby ordered to continue to transmit support payments received from the Respondent
to the Department of Health and Rehabilitative Services until further order of this
Court to the contrary or receipt of Notice to Discontinue Payments from the Department
of Health and Rehabilitative Services, in Which event the support payments shall there-
after be directed and ~y$ble to the person having custody of the child(ren). -
• -DONE AND ORDERED at Fort Pierce, St. Lucie County, Florida, on this
