The undersigned Petitioner alleges:
1, Petitioner's residence is 5104 Oleander Avenue, Fort
Pierce, St. Lucie County, Florida, and Petitioner's post
office address is 5104 Oleander Avenue, Fort Pierce, Florida.

2. Petitioner, whose date of birth is August 19, 1898,
is gl years .of age, and her social security number is 264-01-0781
3. Petitioner, although mentally competent, is incapable
of the care, custody and management of her estate by reason
of age and physical infirmity and hereby voluntarily petitions
the court for the appointment of guardians of her property
pursuant to the applicable provisions of the Florida Statutes.
4. This petition is accompanied by a certificate of a
licensed physician that he has examined the Petitioner and
that the Petitioner is competent to understand the nature of
the guardianship and her delegation of authority. .
