ORDER
THIS CAUSE having come on for hearing on Intervenors Petition
for Modification of final judgment or order on child support and the Court
having reviewed the file and considered the testimony of witnesses and being
otherwise fully advised in the premises, it is
ORDERED AND ADJUDGID that the Petition for Modification of final
~udgment.or order on child support shall be and the sane is hereby granted;

ORDERED AND ADJUDGED that commencing on ~?+2 Q

1980, the Father, DANIEL J. MAYNARD , shall pay to the

Clerk of this Court the sots of $ 25.00 per week each and every ~
week plus $2.00 statutory fee,
which amount shall be remitted swnthly by
the Clerk to the Department of Health and Rehabilitative Services, Child Support
Enforcement Unit, 1317 Winewood Boulevard, Tallahassee, Florida, 32304. All '
payments shall be made in cash, money order or cashiers check. All stoney orders
and cashiers checks shall bear the payees name and Social Security Nusiber and shall
be made payable to the CLERK OF THE CIRCUIT COURT, and sent to:

ORDERID AND ADJUDGED that iU addition to the above-ordered amounts the

until the entire arrearage is fully paid; it is further
ORDERED AND ADJUDGED that the Clerk of this Court shall and he is

responsible parent to the Department of Health and Rehabilitative Services until
g further order of this Court to the contrary or receipt of Notice to Discontinue
Payments from the Department of Health and Rehabilitative Services, in which
event the support payments shall thereafter•be directed and payable to the person
having custody of the child(ren).
DONE AND ORDERED at Fort Pierce, St. Lucie County, Florida, on this
