THIS CAUSE having cone on for trial upon the pleadings filed herein {
and all parties having received proper and timely notice; the Court havins heard
testimony and/or considered the pleadings, papers, affidavits std other papers
filed herein, and being otherwise fully and yell advised in the premises, it is
ORDERED AND ADJUDG® that the winor child(ren)
RODERICK JERMAINE INGRAM, d.o.b. 12/29/70;. RONALD EUGENE INGRAM, d.o.b. 1/7/70
is/are declared to be the legitimate child(ren) of tt~e Defendant
EUGENE INGRAM , and JANICE JOHNSON ~
the natural mother; it is further -
ORDERID AND ADJUDGED that the natural itfs Erandmother
LULA WILLIAMS , shall have custody of the said child(ren)
subject to the Defendant s right of reasonable visitation; it is further

the Defendant/Father shall pay child support for and on behalf of the said child(ren)

Said amount shall be remitted monthly by the Clerk to the Department of Health and
E Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard,
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and is
hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments from
~ tl~e Department of Health and Rehabilitative Services, in which event the support
payments shall thereafter be directed and payable to the aforesaid natural mother or
person-having custody of the child(ren); tt is further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
