testimony and/or considered the pleadings. papers, affidavits and other papers
filed herein, and being otherwise fully and well advistd in the presiaes, it is
ORDE1tED AND ADJUDGED that the winor child(ren)
JUDY ANN MILLER. d.o.b. 12/2/72
is/are declared to be the legitimate child(ren) of the Defendant
JOHN MACKEY and BRENDA MILLER GRIMES
the natural mother; it is further
ORDERID AND ADJUDGED that the natural mother,
BRENDA MILLER GRIMES , shall have custody of the said child(ren)
subject to the Defendant s right of reasonable visitation; it is further

the Defendant/Father shall pay child support for anJ on behalf of the said child(ren)

Said amount shall be remitted wonthly by the Clerk to the Depar paent of Health and
~ Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard,
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGED that the Clerk ~f the -Circuit Court shall and is
hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments from
tl~e Departoent of Health and Rehabilitative Services, in which evtnt the support
payments shall thereafter be directed and payable to the aforesaid natural mother or
person•having custody of the child(ren); it is further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(renj, the DF.PARTi~iENT OF HEALTH AND REHABILITATIVE
SERVICES, riUREAL` OF VITAL STATTSTICS, AMENDMENT UNIT, shall and it is hereby ordertd to:
