and all parties having received proper and timely notice; the Court having heard
testimony and/or considered the pleadings, papers, affidavits and other papers
filed herein, and being other rise fully and yell advised in the premises, it is
ORDERED AND ADJUDGED that the minor child(ren)
LaSHUNE TUSHAY JONES, d.o.b. 1/16/77
is/are declared to be the legitimate child(ren) of the Defendant
WILLIE FRANK JONES , and LESLIE MAE CRENSHAW
the natural mother; it is further
ORDERED AND ADJUDGED that the natural mother,
LESLIE MAE CRENSHAW , shall have custody of the said child(ren)
subject to the Defendant s right of reasonable visitAti~on; it is further
ORDERED AND ADJUDGED that commencing on ~J ~ ~1 ~ .Z~ 1980,
the Defendant/Father shall pay child support for anal on behalf of the said child(ren)
in the amount of $ 50.00 per month plus $2.00 statutory fee

t Said amount shall be remitted monthly by the Clerk to the Department of health and 7
Rehabilitative Services, Child Support Enforcement Unit, 1317 Wine~tood Boulevard,
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and is
! hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments from
the Department of Health and Rehabilitative Services, in t+hich event the support
payments shall thereafter be directed and payable to the aforesaid natural mother or
person-having custody of the child(ren); it !a further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of tl~e above-named child(ren), the DFPAItTMF.NT OF HEALTH AND REHABILITA?IVE
SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT L'NIT, shall and it is hereby ordered to:
