THIS CAUSE having cove on for trial upon the pleadings filed herein
and all parties having received proper and tiiaely notice; the Court having heard
testiuony and/or Considered the pleadings, papers, affida~ita and other papers
filed herein, and being otherwiac fully and yell advised in the presiaea, it is
ORDERED AND ADJUDGED that the minor child(ren)
MARY ANN DAWSON, d.o.b. 6/14/64
is/are declared to be the legitimate child(ren) of the Defendant
MACK DAWSON ~ and MILDRED BOSTIC~
the natural mother; it is further -
ORDEREU AND ADJUDGED that the natural muther,
MILDRID BOSTIC , shall Dave custody of the said child(ren)
subject to the Defendant s right of reasonable visit~t,ion; it is further
ORDERED AND ADJUDGED that commencing on June 16 , 1980,
the Defendant/Father sl?all pay child support for .~nJ on behalf of the said child(ren)
in the amount of $ 20.00 per week plus $2.00 statutory fee
All psys?ent shall be made in cash. money order or cashiers check. All money orders
and cashiers checks shall bear the payees name and Social Security Number and shall
be male payable to the CLERK OF THE CIRCUIT COURT, and sent to:

~ Said amount shall be remitted sionthly by the Clerk to the Departwent of Health and
i Rehabilitative Services, Child Support Enforcement Unit, 1317 Ninewood Boulevard,
Tallahassee, Florida, 32304; it is further
ORDERED AND ADJUDGED that the Clerk ~f the Circuit Court shall and is
hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Paysients fro+s
E ~cl~e Department of Health and Rehabilitative Services, in which event the support
payments shall thereafter be directed and payable to the aforesaid natural mother or
person•having custody of the child(ren); it is further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DF.PARTMF.NT OF HEALTH AND REHABILITATIVE
SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and •i[ is hereby ordered to:
