is/are declared to be the legitisate child(ren) of the Defendant

the natural mother; it is further
ORDERED AND ADJUDGED that the natural Bother,
ANNIE YOUNG shall have custody of the said child(ren)
subject. to the Defendant s right of reasonable visitation; it is further

~ Said amount shall be remitted monthly by the Clerk to the Department of Health and
! Rehabi~~tative Se:~ices, Chip Support Enforcement Unit, 1317 Winewod Boulevard,
Tallahassee, Florida, 32304; it is -further
ORDERED AND ADJUDGED that tt~e Clerk of the Circuit Court shall and is
hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments frog
the Department of Health and Rehabilitative Services, in which event the support
payments shall thereafter be directed and payable to the aforesaid natural Bother or
person-having custody of the child(ren); it is further
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF HEAL'17I AND REHABILITATIVE
SERVICES, BUREAU OF VITAL STATISTICS, AlIENDlIENT UNIT, shall and it is hereby ordered to:
