THIS CAUSE having cove on for trial upon the pleadings filed herein
and all parties having received proper and timely notice; the Court having heard
testiawny and/or considered the pleadings, papers, affidavits and other papers
filed herein, and being otherwise fulty and well advised in the premises, it is
ORDERED AND ADJUDGED that the minor child(ren)
TANESHEYA SHANETTE BANNER, d:o.b. 2/5/80
is/are declared to be the legitimate child(ren) of the Defendant
LORENZO BOLDEN and GWENDOLYN BANNER

the natural mother; it is further _
ORDERID AND ADJUDGED that the natural mother,
GWENDOLYN BANNER , shall have custody of the said child(ren)
subject to the Defendant s right of reasonable visitat-•i~o-n; it is further
ORDERED AND ADJUDGED that commencing on ~J N ~ 1980,
the Defendant/Father shall pay child support for and on behalf of the said child(ren)
in the amount of $ 90.00 per month plus $2.00 statutory fee
All payment shall be made in cash, money order or cashiers check. All money orders •
and cashiers checks shall hear the payees nsa4e and Social Security Number and shall
be made payable to the CLERK OF THE CIRCUIT COURT, and sent to:

Said amount shall be remitted monthly by the Clerk to the Department of Heath and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard,
~ Tallahassee, Florida, 32304; it is further
€ ORDERED AND ADJUDGED that [1?e Clerk ~f the Circuit Court shall and is
hereby ordered to continue to transmit support payments received from the Defendant
until further order of this Court or receipt of Notice to Discontinue Payments from
tl~e Department of Health and Rehabilitative Services, in which event the support
payments shall thereafter be directed and payable to the aforesaid natural mother or
person-having custody of the child(ren); it is further •
ORDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIVE
SERVICES, BUREAU OF VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to:

above-named father's name.
2. remove from the above-named child's
children s birth certificate(s) and enter the above-named father's name.
