pF HEALTN AND RBHAbILITATIVE SERVICES for child support and the Court having found
that said Petitioner is a proper party by virtue of an assisnment of rights to
child support signed Dy the party having custody of the dependent child(ren) and
all parties having received proper notice, it is
ORDERED AND ADJUDGID that the Petition for Child Support shall be and
it is hereby granted; it is further

Said amount shall be remitted monthly by the Clerk to the Department of Health and
~ Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood boulevard,
Tallahassee, Florida, 32304. It is further
` ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and (s)he
is heresy ordered to continue to transmit support payments received from the Respondent
to the Department of Health and Rehabilitative Services until further order of this
~ Court to the contrary or receipt of Notice to Discontinue Paywenta from the Department
~ of Health and Rehabilitative Services, in which event the support payments shall there-
after be directed and payable to the person having custody of the child(ren).
