BEFQRE ME personally appeared DORIS ANN THOMAS now known as DORIS
ANN~MILL5, who being by me first duly sworn says:
1. My name is DORIS ANN MILLS.
2. In April of 1974 I lived with L.V. THOMAS, and we took title
to property described as;
Lot 11, Block 3, SUNRISE PARK SUBDIVISION NO. 1
according to the plat thereof on file in Plat
Book 8, page 42 of the public records of St.
Lucie County, Florida
as Husband and Wife. At that time I was going by the name of DORIS
ANN THOMAS. I was not married at that time to anyone, and I am not
married at this time to anyone. He and I commenced living together
in 1970. -
3. I was not and I am not the Wife of L. V. THOMAS, either
by virtue of marriage or by common law. He and I separated and
ceased living together in the year 1976, and have not lived together
since that year.
