o that LARRY J. HOWE, SR. was in the course of his employment

with the eihployer, NATIONAL STANDARD LIFE INSURANCE COMAPANY,
o when he was injured at the time and place alleged in the
r Complaint filed in this cause, and LIBERTY MUTUAL INSURANCE COM-
S PANY has paid to the Plaintiff, LARRY J. HOWE, SR., various

4 o sums of money for Workers' Compensation and medical benefits and
I
that it is probable that future compensation and medical benefits
EI under the Florida Workers' Compensation Act will be paid to
i the Plaintiff.

WHEREFORE, LIBERTY MUTUAL INSURANCE COMPANY claims a lien
upon any recovery by the Plaintiff for the f~fll amount of benefit
previously paid to the Plaintiff under Florida's Workers' Compen-
~ sation Act, as well as the full amount of the benefits to be
paid in the future under the provisions of the Act.
