MOTION FOR ORDER ALLOWING
CUSTODIANS TO APPROVE SURGERY FOR MINOR
The Petitioners herein, Kenneth Roger Moore and Edna
Lucille Moore, by and through their undersigned attorney,
hereby move .this Court for an Order allowing Dr. William
King Stubbs, M.D., to perform a tonsillectomy and adenoidectomy
on the minor, Mysti ReneB Moore n/k/a Mysti ReneB Arvo
and as~grounds therefore would show unto the Court that as
per the attached letter from Dr. Stubbs, it is necessary that
- said surgery be performed on the minor and in order for such
surgery to be undertaken, it is necessary for the Doctor
and hospital to obtain the necessary consents. Attached
hereto is the approval of the Florida Department of Health
and Rehabilitative Services for such operation. Petitioners
have also contacted the natural mother-of the minor who
indicated her consent.

j WHEREFORE, the Petitioners request that this Court
~ enter an Order allowing them to approve such surgery on said
minor and execute any and all necessary releases in order to
allow such surgery to go forward.

THIS CAUSE having come to be heard upon the Motion of
the Petitioners, Kenneth Roger Moore and Edna Lucille Moore
for an Order authorizing Dr. William King Stubbs to perform
tonsillectomy and adenoidectomy surgery on the minor herein
Mysti Renee Moore n/k/a Mysti Renee Arvo as per a letter from
