same, and also the approval of the Florida Department of
Health and Rehabilitative Services and also having received a
telephone call from the natural mother of said minor this
date at which time she indicated her consent to such operation,
-it is thereupon
ORDERED AND ADJUDGED that William King Stubbs, M.D.
is hereby authorized to perform necessary surgery on the minor,
Mysti Renee Moore n/k/a Mysti Renee Arvo in accordance with.
his letter dated June 12, 1980 a copy being attached hereto
and the Petitioners, Kenneth Roger Moore and Edna Lucille
Moore, are hereby authorized to approve such surgery on behalf
of said minor and to execute all necessary releases and documents
required in order to effectuate such surgery.
