PETITION FOR VOLUNTARY GUARDIANSHIP
The undersigned petitioner alleges:
1. Petitioner's residence is Abbiejean Russel Care Center, and
petitioner's post office address is ?00 South 29th Street, Fort Pierce, ~
Florida (33450).
2. Petitioner is 61 years of age, and her social security
number is 115-10-2606.

3. Petitioner, although mentally competent, is incapable of the
care, custody and management of her estate by reason of physical
infirmity and voluntarily petitions the court for appointment of a
guardian of her property pursuant to the applicable provisions of the
Florida Statutes.
4. This petition is accompanied by a certificate of a licensed
physician that he has examined petitioner and that petitioner is

competent to understand the nature of the guardianship and her delegation
of authority.
5. The approximate value and description of the petitioner's '

property are:
(1) Advancements from George T. Balling Trust under deed
with United States Trust Company, dated November 27,
1956, for room and board at Abbiejean Russel Care Center.
