THIS CAUSE having comae on for hearing on the petition of the DEPARTMENT
OF HEALTH AND REHABILITATriB SERVICES for child support and the Court having found
that said Petitioner is a proper party by virtue of an assigasent of rights to
child support signed by the party having custody of the dependent child(ren) and
all parties having received proper notice, it is
ORDERID AND ADJUDGED that the Petition for Child Support shall be and
it is hereby granted; it is further
ORDERID AND ADJUDGED that com~eencing on ~ ~ ~ y l ~ 1980,

Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Eoulevard,
Tallahassee, Florida, 32304. It is further
ORDERED AND ADJUDGED that the Clerk of the Circuit Court shall and (s)he
is hereby ordered to continue to transwit support paywenta received from the Respondent
to the Department of Health and Rehabilitative Services until further order of this
Court to the contrary or receipt of Notice to Discontinue Pays~ents from the Department
R of Health and Rehabilitative Services, in which event the support papsents shall there-
after be directed and payable to the person having custody of the. child(ren).
