AFFIDAVIT OF OPERATORS USING TRADE NAME

COUNTY OF ST. LUCIE
BEFORE ME, the undersigned authority, this day
personally appeared FRANR DE T80MAS, JR., President of FRANK DE ,
CONSTRUCTION SERVICES, INC., who being by me first duly sworn,
deposes and says:
- That this Affidavit is made in compliance with
Section 865.09 of the Florida Statutes.
That the Affiant composes all of the persons
interested in-the business enterprise known and doing business
as: FLORIDA CONSTRUCTION SERVICES, .INC., located and operating
at the following address:

and that the extent of the interest of each Affiant in said
