AMENDID ORDER
THIS CAUSE having cave on for hearing on Intervenor's Petition
for Modification of final judgment or order on child support and the Court f
having reviewed the file and considered the testimony of witnesses and being
otherwise fully advised in the premises. it is ~
ORDERED AND ADJUDGED that the Petition for Modification of final
judgttsent or order on child support shall be and the same is hereby granted;
it is further ~
ORDERED AND ADJUDGED that commencing on August 8

1980. the Father, BARRY KEITH DAVIS _ , shall pay to the {
Clerk of this Court the sum of $ 15.00 per month each and every
month , plus $2.00 statutory fee, until the AFDC is reimbursed in the.
amount of $543.00 which amount shall be remitted monthly by
the Clerk to the Department of Health and Rehabilitative Services, Child Support
Enforcement Unit, 1317 Winewood Boulevard, Tallahassee, Florida, 32304. All
payments shall be made in cash, money-order or cashiers check. All money orders
and cashiers checks shall bear-the payees name and Social Security Number and shall
- be made payable to the CLERK OF THE CIRCUIT COURT, and sent to:

it is further -
~ ORDERID AND ADJUDGED that in addition to the aboverordered asounts the

as set forth herein as and for arrearagea or past unpaid child support which is due `'s

until the entire arrearage is fully paid; it is further
ORDERED AND ADJUDGED that the Clerk of this Court shall and he is
hereby ordered to continue to transmit support payments received from the
responsible parent to the Department of Health and Rehabilitative Services until
further order of this Court to the contrary or receipt of Notice to Discontinue i
Payments from the Department of Health and Rehabilitative Services, in which
event the support payments shall thereafter be directed and payable to the person
having custody of the-child(ren).
