ORDER
THIS CAUSE having come on for hearing on Intervenor's Petition
for Modification of final judgment or order on child support and the Court
having reviewed the file and considered the testimony of witnesses and being
otherwise fully advised in the premises, it is
ORDERED AND ADJUDGID that the Petition for Modification of final
judgment or order on child support shall be and the same is hereby granted;
it is further
ORDERED AND ADJUDGED that cosatiencing on
1980, the Father, WILLIAM EDWARD LEWIS , shall pay to the
Clerk of this Court the sum of $ 70.00 per week each and every
week plus $2.00 statutory fee,
which amount shall be remitted monthly by
the Clerk to the Department of Health and Rehabilitative Services, Child Support
~ Enforcement Unit, 1317 Winewood Boulevard, Tallahassee, Florida, 32304. All
payments shall be made in cash, money-order or cashiers check. All money orders
and cashiers checks shall bear the payees name and Social Security Number and shall
i be made payable to the CLERK OF THE CIRCUIT COURT, and sent to:

responsible parent to the Department of Health and Rehabilitative Services until
further order of this Court to the contrary or receipt of Notice to Discontinue
Payments from the Department of Health and Rehabilitative Services, in which
~ event the support payments shall thereafter be directed and payable to the person `
having custody of the child(ren).
